At Changing Lane’s Foundation (CLF), Inc., our Team remembers the challenges thrust before us when we were diagnosed with breast cancer. Therefore, we strive to avail to others what wasn’t readily available to us. We’ve seen the curve balls, and through our collective experiences, now know how to hit a curve ball.
CLF’s Mission is to serve surviving Breast Cancer patients and survivors while navigating and assisting them in finding financial resources, and health and wellness information so they are not subjugated or stressed during difficult times.
Strive to support Breast Cancer communities and educate those we come in contact with about early detection of Breast Cancer.
ELIGIBILITY REQUIREMENTS FOR ASSISTANCE
Applicant must reside in either Gwinnett, Fulton, or Dekalb Counties, complete the Assistance Registration form, and provide the following documents via email to: [email protected]
Breast Cancer Diagnosis Letter dated 90 days or less, on Physician Letterhead, and signed by either the Physician, Nurse, or Social Worker
Copy of current Rental Lease or Mortgage Statement
Copies of past due Rent or Mortgage Notice, and/or Utility Bill(s)
Copy of current Driver’s License or State ID
