If you are currently in a sub-acute or hospital setting and seeking recovery options, please ask your caseworker or social services representative to provide our Admissions Office with your latest medical records. Your caseworker will collaborate directly with us to ensure that all necessary documentation is submitted.
For those seeking recovery at A.G. Rhodes and living at home or in an independent or assisted living facility, your primary care physician will need to send specific documentation to our Admissions Office.

Face Sheet (including insurance information)
Level 1 or Level 2 form (available from your physician’s office or we can provide a copy if needed)
DMA-6 form (available from your physician’s office)
Physician orders, Medication Administration Record (MAR), and medication listing
Three days of nurse’s notes
Current history and physical
Therapy notes (Physical Therapy, Occupational Therapy, Speech Therapy)
Most recent chest x-ray or Tuberculosis (TB) test results
Additional documentation as needed and if applicable
